No.: WV0084425 Applicant: PAGE-KINCAID PSD PO BOX 165 KINCAID, WV 25119 Location: KINCAID, FAYETTE COUNTY Latitude: 38:03:38 Longitude: 81:15:56 Receiving Stream: LOOP CREEK/BEARDS FORK Activity: To operate and maintain an existing 48,000 gallons per day sewage collection and aerated lagoon treatment system. These facilities serve approximately 987 persons or equivalents in the communities of Page and Kincaid in the Page-Kincaid PSD and discharge treated and disinfected wastewater, via Outlet No. 001, to Loop Creek (approximately 8.9 miles from its mouth) of the Kanawha River. Also to operate and maintain an existing 16,000 gallons per day extended aeration package wastewater collection and treatment system. These facilities serve approximately 288 persons or equivalents in the community of Beards Fork in the Page-Kincaid PSD and discharge treated and disinfected wastewater, via Outlet No. 002, to Beards Fork Creek (approximately 1.8 miles from its mouth) of Loop Creek of the Kanawha River. Also to operate and maintain an existing 60,000 gallons per day wastewater collection and treatment system. These facilities serve a population equivalent of approximately 900 persons in the Ingram Branch, Wriston, and Hamilton area of the Page-Kincaid PSD and discharge treated and disinfected effluent, via Outlet No. 003, to Loop Creek (Approximately 13.9 miles from its mouth) of the Kanawha River. Also, to accept non-domestic wastewater from the water treatment plant for subsequent treatment and disposal at Outlet No. 001. Also, to incorporate the requirements relative to Title 33, Series, 2, of the West Virginia Legislative Rules for the processing and disposal of sewage sludge generated by the wastewater treatment plants. Sewage sludge generated and/or processed at the permittee’s facility shall be disposed of at a sanitary landfill by placing the sewage sludge in the landfill cell. Tier 1 protection is provided for the uses specified in Title 47, Series 2, Section 6 for Outlet Nos. 001, 002, and 003. Any interested person may submit written comments on the draft permit and may request a public hearing by addressing such to the Director of the Division of Water and Waste Management within 30 days of the date of the public notice.
