UNITED DAIRY SNC NOTICE The following industrial user has demonstrated Significant Noncompliance as defined by Section 118-212 (a) Items (6), (7), and (8) of Code of Ordinances of the City of Charleston WV, Chapter 118, Article III – Division 6. United Dairy discharged wastewater in violation of Industrial User Permit 109, as follows: 1. Part II.A: failure to submit monthly discharge monitoring reports on or before the required due dates. 2. Part II.B: failure to notify the Charleston Sanitary Board within 24 hours of violation of effluent limitations and re-sample and analyze for the pollutant(s) in violation of effluent limitations. 3. Part II.G: failure to notify the Charleston Sanitary Board of potential problems that might cause problems for the POTW. 4. Part III.B: failure to properly operate and maintain all facilities and treatment systems used to treat or otherwise control discharges in order to achieve permit compliance. 5.
