That at the time of this petition, Petitioner has been a bona fide resident of Putnam County, WV for at least one year prior to the filing of the petition; 2. That Petitioner wishes to change his name from Michael William Hill to Michael William Azzarone, to reflect the last name of his step-father who reared him from a child and supported him through his lifetime; 3. Petitioner is a veteran of the United States Air Force; 4. Date of birth, May 24, 1990 and SSN xxx-xx-1707; 5. Petitioner wished for the change from last name Hill to Azzarone; 6. The reason for the change is NOT for purposes of avoiding debt or creditors; 7. Petitioner is NOT a registered sex offender pursuant to any state or federal law; 8. Petitioner does not seek the name change for purposes of avoiding any state or federal law regarding identity; 9. The name change is not sought for any improper or illegal purpose; 10. Petitioner is NOT a convicted felon in any jurisdiction; 11. The name change is not for any purpose of evading detection, identification or arrest by any local, state or federal law-enforcement agency; 12. There are no reasons for protection of identity for safety reasons. PRAYER FOR RELIEF 1. That Petitioner be granted a name change from Michael William Hill to Michael William Azzarone. 2. That an ORDER be established from the Court and sent to W.Va. office of Vital Statistics, which orders such name change on the birth certificate of Petitioner. 3.
