No.: WV0024562 Applicant: WAYNE TOWN OF 10328 RT 152 RD WAYNE WV 25570 Location: WAYNE WAYNE COUNTY Latitude: 38:13:46 Longitude: 82:26:26 Receiving Stream: TWELVEPOLE CREEK Activity: To operate and maintain an existing combined wastewater collection system and an existing 0.15 million gallons per day wastewater treatment plant. These facilities are designed to serve a population equivalent of approximately 1 500 persons in the Town of Wayne and environs and discharge treated wastewater through Outlet No. 001 to Twelvepole Creek approximately 27 miles from its mouth of the Ohio River. Also to operate and maintain disposal systems best management practices and the nine (9) minimum controls for the direct discharge of sanitary wastewater and storm water from Combined Sewer Overflow (CSO) Outlet Nos. C004 C005 and C101 to Twelvepole Creek of the Ohio River. These CSO outlets are permitted to discharge only when the hydraulic capacity of the collection and/or treatment system is exceeded during wet weather events. Also to incorporate the requirements relative to Title 33 Series 2 of the West Virginia Legislative Rules for the processing and disposal of sewage sludge generated by the wastewater treatment plant. Sewage sludge generated and/or processed at the permittee’s facility shall be disposed of at a sanitary landfill by placing the sewage sludge in the landfill cell and/or by a registered septage hauler.
