WELL DRILLING PERMITS, pursuant to West Virginia
Code Section 22-6A 10(e).
EQT Production Company, 2200 Energy Drive,
Canonsburg, PA 15317, is applying for permits for
natural gas horizontal wells at the following locations:

These wells are located in the West Virginia Fork Fish Creek Watershed, Hundred Quadrangle, Church
District, Wetzel County, West Virginia which disturbs
three acres or more of surface excluding pipelines,
gathering lines and roads or utilizes more than two
hundred ten thousand gallons of water in any thirty-day
period.

Charleston, WV 25304, (304-926-0450). Full copies or
scans of the proposed permit application will cost
$15.00, whether mailed or obtained at DEP
headquarters.
