PARENTAL CONSENT TO STEPPARENT ADOPTION

Personally appeared before the undersigned notary public, duly authorized to administer oaths in the State of Georgia, ELIZABETH JANE GUERCIA, who, after being duly sworn, deposes and states the following:

I, the undersigned, hereby consent that my spouse, LEE RICHARD GUERCIA, adopt my daughter, EVELYN PRESLEY HAINES, whose date of birth is January 19, 2018, and in so doing I in no way relinquish or surrender my parental rights to the child. I further acknowledge service of a copy of the petition for adoption of the child as filed on behalf of my spouse, and I hereby consent to the granting of the prayers of the Petition to Adopt Evelyn Presley Haines and Termination of Parental Rights. I also waive all other and further service and notice of any kind and nature in connection with the proceedings.

The biological father, VERNON RAY HAINES, has not legitimated the child. However, the biological father is identified on the birth certificate for the child, and the biological mother has identified the biological father as the father of the child at issue. Based on information and belief, VERNON RAY HAINES is located in the State of West Virginia, and service attempts will be made therein.
