The Petitioner, the West Virginia Department of Health and Human Resources has filed a Petition that may result in the termination of parental rights of NIKKI KOFFEL, biological mother and primary custodian of infant R.C., whose date of birth is August 7, 2015 and UNKNOWN FATHER of infant R.C., whose date of birth is August 7, 2015; natural child of NIKKI KOFFEL
The Petitioner alleges NIKKI KOFFEL, biological mother and primary custodian of infant R.C., whose date of birth is August 7, 2015 and UNKNOWN FATHER of infant R.C., whose date of birth is August 7, 2015, has abandoned and /or failed and neglected to provide for the care, custody and control of infant, R.C., whose date of birth is August 7, 201 5, and has made no effort to provide for said care of infant, or maintain the infant in any way from that time until present. It is ORDERED NIKKI KOFFEL, biological mother and primary custodian of infant R.C., whose date of birth is August 7, 2015 and UNKNOWN FATHER of infant R.C., whose date of birth is August 7, 2015 are notified that they have been ORDERED to appear in the Courtroom of the Honorable James P. Mazzone, Judge of the Circuit Court of Ohio County, West Virginia, Fifth Floor, Ohio County Courthouse, Wheeling, West Virginia on the 16th day of March, 2016 at 10:00 a.m. for a Adjudication Hearing and on the 28th day of March, 2016 at 10:30 a.m. for a Disposition Hearing and object or make other defense to the petition; otherwise, parental rights NIKKI KOFFEL, biological mother and primary custodian of infant R.C., whose date of birth is August 7, 2015 and UNKNOWN FATHER of infant R.C., whose date of birth is August 7, 2015 may be terminated.
