PUBLIC NOTICE OFAPPLICATION FOR NATURAL GAS HORIZONTAL WELL DRILLING PERMITS,
pursuant to West Virginia Code Section 22-6A-10(e).
EQT Production Company, 400 Woodcliff Drive, Canonsburg, PA 15317, is applying for permits for
natural gas horizontal wells at the following locations:
Sizemore 3H, located at approximately UTM NAD 83 Easting 540416.04 UTM NAD 83 Northing 4388550.01;
Sizemore 5H, located at approximately UTM NAD 83 Easting 540416.88 UTM NAD 83 Northing 4388552.94;
Sizemore 7H, located at approximately UTM NAD 83 Easting 540417.72 UTM NAD 83 Northing 4388555.87;
Sizemore 9H, located at approximately UTM NAD 83 Easting 540418.56 UTM NAD 83 Northing 4388558.80;
Sizemore 11H, located at approximately UTM NAD 83 Easting 540419.39 UTM NAD 83 Northing 4388561.73;
Sizemore 13H, located at approximately UTM NAD 83 Easting 540420.23 UTM NAD 83 Northing 4388564.66;
Sizemore 15H, located at approximately UTM NAD 83 Easting 540421.07 UTM NAD 83 Northing 4388567.59;
Sizemore 17H, located at approximately UTM NAD 83 Easting 540421.91 UTM NAD 83 Northing 4388570.52

All wells are located in the Tributary of Sugar Run Watershed, Littleton Quadrangle, Center District, Wetzel County, West Virginia which disturbs three acres or more of surface excluding pipelines, gathering lines and roads or utilizes more than two hundred ten thousand gallons of water in any thirty-day period.

Copies of the proposed permit applications may be
reviewed at the WV Department of Environmental
Protection headquarters, located at 601 57th Street, SE, Charleston, WV 25304, (304-926-0450). Full copies or scans of the proposed permit application will cost $15.00, whether mailed or obtained at DEP
headquarters.
