ADVERTISEMENT FOR ARCHITECTURAL / ENGINEERING SERVICES FOR THE SUSTAINABLE LIVING INITIATIVE BOONE MEMORIAL HOSPITAL is accepting Statements of Interests from qualified persons or firms to provide their Sustainable Living Initiative which consists of a Farmers Market, Commercial Greenhouse and a Healthy Grille teaching and farm-to-table restaurant. This project consists of architectural, engineering and construction management services which will include a cost analysis, design services including drawings, preparation of plans and specifications, bid documents, construction administration/management and observation services for the Madison Farmers Market, Greenhouse and Healthy Grille project. Boone Memorial Hospital reserves the right to select an individual person or firm to complete this project based its ranking. The Selection Committee will select a firm and negotiate the contract. The Selection Committee will include representatives from the hospital that have direct knowledge of the project and/or have detailed knowledge of construction projects in general. Procurement of said services would be in accordance with Chapter 5G-1-3 of the WV Code. Interested persons or firms must submit three (3) copies of all requested information by October 18, 2021 at noon to Kevin Fowler, Strategic Planning Consultant, 701 Madison Avenue, Madison, WV 25130 to receive consideration. Additionally, interested entities will need to send an email copy of their response to: [email protected]. Proposals must include: 1) Experience of the firm with public financed projects. 2) Experience in design and management of this type of project. 3) Qualifications of personnel directly involved in the project. 4) Related prior experience. 5) Workload to fulfill the commitments. The selected person or firm will be required to comply with Title VI of the Civil Rights Act of 1964, Executive Order 11246, Section 109 of the Housing and Urban Development Act of 1974, Section 3 of the Housing and Urban Development Act of 1968, Conflict of Interest Statement and Access to Records provisions, Architect’s Certification and all other requirements as they relate to this project. Additionally, the selected person or firm will need to follow the specific guidelines and requirements and provide the deliverables therein of Economic Development Form – ED 900. The Selection Committee will interview and select the most qualified firm(s). The Boone Memorial Hospital will afford full opportunity to minority business enterprises to submit a show of interest in response to this request and will not discriminate against any firm or individual on the grounds of race, creed, color, sex, age, handicap or national origin.
