No.: WV0000086 Applicant: ALTIVIA SERVICES, LLC 250 CARBIDE RD. INSTITUTE, WV 25064 Location: INSTITUTE, KANAWHA COUNTY Latitude: 38:23:11 Longitude: 81:47:55 Receiving Stream: KANAWHA RIVER Activity: To accept wastewater from US Methanol into the Altivia wastewater treatment plant which ultimately discharges through Outlet 004 to the Kanawha River at Milepoint 48. Also to accept cooling water and untreated storm water from US Methanol which ultimately discharges through Outlet 002 to the Kanawha River at Milepoint 48.62. A Tier 2 and Tier 1 antidegradation review was conducted and Tier 2 and Tier 1 protection are afforded for the additional wastewater being discharged through Outlet 002 and 004 for the uses specified in 47 CSR 2, Section 6. Business conducted: Manufacturer of organic and inorganic chemicals and glutaraldehyde. Implementation: NA On the basis of review of the application, the “Water Pollution Control Act (Chapter 22, Article 11-8(a)),” and the “West Virginia Legislative Rules,” the State of West Virginia will act on the above application. Any interested person may submit written comments on the draft permit modification and may request a public hearing by addressing such to the Director of the Division of Water and Waste Management within 30 days of the date of the public notice. Comments received within this period will be considered prior to acting on the permit modification application. The Director shall hold a public hearing whenever a finding is made, on the basis of requests, that there is a significant degree of public interest on issues relevant to the Draft Permit Modification(s).
