Sealed Bids for the Camden Family Health Beaver Location will be received by the Camden-on-Gauley Medical Center, Inc., located at 10003 Webster Road, Craigsville, West Virginia, until 1:00 p.m., E.S.T., on August 8, 2019 for furnishing labor and materials and performing all Work set forth in the Contract Documents prepared by The Thrasher Group, Inc. Immediately following the scheduled closing time for the reception of Bids, all proposals which have been submitted in accordance with the conditions of the Contract Documents will be publicly opened and read aloud.
The approximate quantities of Work to be Bid upon are as described as follows:
The Work of the project generally consists of renovation of an existing school building into medical clinic space including general care, Behavioral Health, and Dental providers. Renovation work consists of selective demolition of existing masonry partitions, gypsum and lay-in ceilings, and floor coverings. Renovations and upgrades to existing plumbing, mechanical systems, and electrical systems shall also be part of the work. Improvements shall include new partitions of steel studs and gypsum board, new floor coverings, new ceilings, new light fixtures, new plumbing fixtures, new HVAC systems, specialties associated with provider equipment, and other Work indicated in the Contract Documents.
The Work will be substantially completed within 180 consecutive calendar days. Final completion will be 30 days thereafter. Liquidated damages shall be $500.00 per day.

Complete sets of Bidding Documents may be obtained from the office of The Thrasher Group, Inc., 600 White Oaks Boulevard, PO Box 940, Bridgeport, WV 26330, for the following costs:
— Bidding Documents issued as hard copy drawings and hard copy specifications are available for $50 per set.
— Bidding Documents issued as digital drawings and digital specifications on a C.D., are available for $25 per set.
Amounts paid are not subject to refund.
A two envelope system will be used.
Envelope No. 1 must have the following information presented on the front:

Bid for Camden Family Health Beaver Location
Received by the Camden-on-Gauley Medical Center, Inc.
Envelope No. 2 labeled “Bid Proposal” shall also be placed inside of Envelope #1.
Envelope No. 1 will be opened first and the Bid Opening Requirement items checked for compliance as outlined on the Bid Opening Checklist on page BOR – 1 of these contract documents. If such documents are found to be in order, Envelope No. 2 “Bid Proposal”, will then be opened and will be publicly read aloud. If the documents required to be contained in Envelope No. 1 are not in order, Envelope No. 2 “Bid Proposal” will not be opened and the Bid will be considered non-responsive and will be returned to the Bidder.
A Bidder may not withdraw his Bid for a period of thirty (30) days after the date set for the opening of Bids.
Bids shall be accompanied by a Bid Bond payable to the Camden-on-Gauley Medical Center, Inc., an amount equal to five percent (5{4ceb532c6f579389df471c6c1e832caf2346b74dc60fcbf6aabd4d29df3baf9c}) of the Total Bid.
Contractor is required to provide the Drug Free Workplace Certification in accordance with West Virginia Code 21-1D-5.
All Bidders are required to have a West Virginia Contractor’s License per Section 21-11-11 of the West Virginia Code.
Bids received after the scheduled closing time for the reception of Bids will be returned unopened to the Bidders.
The Camden-on-Gauley Medical Center, Inc. reserves the right to reject any and all Bids.
A MANDATORY pre-bid conference will be held at the former Beaver Creek Elementary School, located at 16414 Webster Road, Craigsville, West Virginia, on July 25, 2019, at 10:30 a.m., E.S.T.
