THRASHER PROJECT #101-060-0594
Sealed Bids for Contract MVA’s Fairmont Clinic – Lobby Renovations will be received by Monongahela Valley Association of Health Centers, Inc. 1322 Locust Avenue, Fairmont, Marion County, West Virginia, until 2:00 p.m., L.P.T., on Tuesday, June 30, 2015, for furnishing labor and materials and performing all Work set forth in the Contract Documents prepared by The Thrasher Group, Inc. Immediately following the scheduled closing time for the reception of Bids, all proposals which have been submitted in accordance with the conditions of the Contract Documents will be publicly opened and read aloud.
The approximate quantities of Work to be Bid upon is as described as follows:
In general the Contractor is to provide all labor, materials, equipment, fees, insurance, taxes and similar contractor expenses as required for the renovation to the main lobby of the MVA Fairmont Clinic in Fairmont, WV.
The Work will be substantially completed within 120 days after the date when the Contract Times
commence to run, and completed and ready for final payment within 134 days after the date when the Contract Times commence to run. Liquidated damages shall be $150.00 per day.

Complete sets of Bidding Documents may be obtained from the office of The Thrasher Group, Inc., 600 White Oaks Boulevard, P. O. Box 940, Bridgeport, WV 26330, for the following costs:
• Bidding Documents issued as hard copy drawings and hard copy specifications are available for $35 per set.
• Bidding Documents issued as digital drawings and digital specifications on a C.D. are available for $25 per set.
Amounts paid are not subject to refund.
A two envelope system will be used.
Envelope No. 1 must have the following information presented on the front:
Name and address of Bidder
Bid on: MVA’S Fairmont Clinic Lobby Renovations
Received by the: Monongahela Valley Association of Health Centers, Inc.
Envelope No. 2 labeled “Bid Proposal” shall also be placed inside of Envelope #1.
Envelope No. 1 will be opened first and the Bid Opening Requirement items checked for compliance as outlined on the Bid Opening Checklist on page BOR – 1 of these contract documents. If such documents are found to be in order, Envelope No. 2 “Bid Proposal”, will then be opened and will be publicly read aloud. If the documents required to be contained in Envelope No. 1 are not in order,
Envelope No. 2 “Bid Proposal” will not be opened and the Bid will be considered non-responsive and will be returned to the Bidder.
A Bidder may not withdraw his Bid for a period of ninety (90) days after the date set for the opening of Bids.
Bids shall be accompanied by a certified check or Bid Bond payable to the owner, an amount equal to five percent (5{4ceb532c6f579389df471c6c1e832caf2346b74dc60fcbf6aabd4d29df3baf9c}) of the base Bid.
Bidders must hold a current West Virginia contractor’s license on the date of Bid Opening.
Bids received after the scheduled closing time for the reception of Bids will be returned unopened to the Bidders.
The Owner reserves the right to reject any and all Bids.
The project is funded with a $247,675.00 grant award received from the Affordable Care Act, Patient Centered Medical Home – Facility Improvements Grant Program as administrated through the U.S. Department of Health and Human Services, Health Resources and Services Administration (HRSA) which grant accounts for 100{4ceb532c6f579389df471c6c1e832caf2346b74dc60fcbf6aabd4d29df3baf9c} of the total Project cost budgeted at $247,675.00. Total project costs include: administrative expense, architectural and engineering fees, equipment, and construction.
A pre-bid conference will be held at the MVA Fairmont Clinic located at 1322 Locust Avenue,
Fairmont, Marion County, West Virginia, on Tuesday, June 16, 2015, at 3:00 p.m., L.P.T.
