Box 707, Jerry Fork Road (Route 19/21), Summersville, WV 26651. This application will assign Republic Energy, LLC, 4101 Slate Drive, Scarbro, WV 25917 as an operator on the permits(s) named above.
Comments on the application shall be in writing and shall identify the applicant and application number and should be sent to the DEP at the address above.
