PUBLIC SERVICE COMMISSION OF WEST VIRGINIA CHARLESTON Entered by the PUBLIC SERVICE COMMISSION OF WEST VIRGINIA, in the City of Charleston on the 1st day of October 2015. CASE NOS. 15-1416-MC-AC, 15-1417-MC-AC, 15-1418-MC-AC and 15-1419-MC-AC PROFESSIONAL TRANSPORTATION, INC. a corporation. Application to amend P.S.C. M.C. Certificate Nos. 6878, 7278, 7299 and 7398 to include Kanawha, Jackson, Roane, Mason, Wirt, Ritchie, Pleasants, Logan, Raleigh, Summers, Greenbrier, and Fayette Counties for specialized limousine service. COMMISSION ORDER On August 27, 2015, Professional Transportation, Inc. filed an application for the amendment of P.S.C. M.C. Certificate Nos. 6878, 7278, 7299 and 7398. The proposed amendment is as follows: to operate as a common carrier by motor vehicle in the transportation of passengers in specialized limousine service, through the use of specialized limousines in specialized multi passenger van service, to and from dialysis treatment, physician offices, hospitals and other health care facilities, such transportation being restricted to those passengers who will receive, or have received, services at such offices, hospitals and facilities, between points and places in Kanawha, Jackson, Roane, Mason, Wirt, Ritchie, Pleasants, Logan, Raleigh, Summers, Greenbrier and Fayette Counties, West Virginia, on the one hand, and points and places in West Virginia on the other hand, subject to the following exclusion: Excluding the transportation of those individuals who have medical needs greater than those of the average population and who are expected to require the presence of a trained emergency medical technician during transport. The Commission does not have jurisdiction over the provision of emergency medical services, which lies with the Commission of the West Virginia Bureau of Public Health. Rates: P.S.C. M.C. Certificate No. 6878 $1.00 per loaded mile $0.50 per unloaded mile Minimum Charge: $30.00 per trip, including trips called, but not used Waiting Time: First 30 minutes: Free $0.25 for each minute thereafter For commercial customers who desire dedicated twenty-four hour-a-day service Professional Transportation, Inc., may charge an hourly rate of $11.00. P.S.C. M.C. Certificate No. 7299 $30.00 per hour or $1.00 per mile loaded $.50 per mile unloaded Minimum Charge: $15.00 per trip, including trips called, but not used Waiting Time: $ 0.25 per minute after the first half hour Stretch Limousine: $65.00 per hour Professional Transportation, Inc. shall inform its customers whether it will be charging the hourly or mileage rate prior to commencing the trip. For commercial customers who desire dedicated twenty-four hour-a-day service Professional Transportation, Inc., may charge an hourly rate of $11.00. P.S.C. M.C. Certificate No. 7398 $1.00 per mile loaded $0.50 per mile unloaded Minimum charge: $15.00 per trip. including trips requested, but not used Once the customer orders a trip and a vehicle is dispatched, the customer will be charged the applicable rates for the trip even if the trip is subsequently cancelled, or passengers or property are not transported on the trip. Waiting time for all trips: First 30 minutes per trip free $0.25 per minute after the first half hour Dedicated vehicle rates: $10.80 per hour per vehicle or $8,195.00 per month per vehicle Mileage rates must be paid to carrier even if total paid per vehicle for a particular day exceeds daily dedicated-vehicle rate. Certificate No. 7278 $1.00 per loaded mile $0.50 per unloaded mile Minimum charge per trip: $15.00 $17.00 per hour (loaded or unloaded when the vehicle is in motion) Waiting time for all trips: $0.30 per minute after the first 30 (free) minutes Determination of rate: Prior to commencing a trip, the carrier shall inform its customers whether it will charge the hourly rate or the mileage rate. For cancelled trip: $15.00 or charge for mileage, whichever is greater $1.85 for the first 0.5 mile $0.20 for each 0.1 mile thereafter $0.70 for each additional passenger over two (2) in the same party $0.20 per minute for waiting time after five (5) minutes $0.36 per package IT IS THEREFORE ORDERED that Professional Transportation, Inc. give notice of the filing of the application, by publishing a copy of this order once in a qualified newspaper as provided in West Virginia Code c59-3- 1 et seq, published and of general circulation in Kanawha, Jackson, Roane, Mason, Wirt, Ritchie, Pleasants, Logan, Raleigh, Summers, Greenbrier, and Fayette Counties, making due return to this Commission of proper certification of publication immediately after publication. Anyone desiring to protest and/or intervene should file a written protest and/or notice of intervention within ten days following the date of this publication unless otherwise modified by Commission order. If no protests are received, the Commission may waive formal hearing and render its decision on the application based on the evidence submitted with the application and the Commission’s review thereof. Requests to intervene must comply with the Commission’s rules on intervention set forth in the Commission’s Rules of Practice and Procedure.
