PETITION FOR ADOPTION
I.
Petitioner, Steven Mark Lilly, whose date of birth is July 28, 1970, represents unto the Court that he is a citizen and resident of Raleigh County, West Virginia, residing in Beckley, Raleigh County, West Virginia, and is the maternal grandfather of the children herein.
II.
Petitioner, Pamela Lynn Lilly, whose date of birth is May 5, 1975, represents unto the Court that she is a citizen and resident of Raleigh County, West Virginia, residing in Beckley, Raleigh County, West Virginia, and is the maternal grandmother of the children herein.
III.
Petitioners state that they are seeking to adopt Nathan Isaiah Steele, Jr. who was born on August 29, 2018 in Raleigh County, West Virginia; and Madilynn Kathleen Steele, who was born on July 20, 2019.
IV.
The biological mother of Nathan Isaiah Steele, Jr., and Madilyn Kathleen Steele is Hannah Lynn Steele.
V.
The biological father of Nathan Isaiah Steele, Jr., and Madilyn Kathleen Steele is Nathan Isaiah Steele.
VI.
The minor children are currently residing with the Petitioners and have lived with them since December 17, 2019.
VII.
That by Order of the Circuit Court of Fayette County, West Virginia, dated November 5, 2020, attached hereto, the Honorable Thomas H. Ewing, Ordered the paternal rights of Nathan Isaiah Steele and Hannah Lynn Steele as to their children, Nathan Isaiah Steele, Jr., and Madilynn Kathleen Steele be terminated and forever barred. Said Orders are attached hereto.
VIII.
That the State of West Virginia, Department of Health and Human Resources has consented to the adoption of the said children, Nathan Isaiah Steele, Jr., and Madilynn Kathleen Steele, and said Consent is attached hereto.
IX.
That a Home Study has been completed by Children’s Home Society of West Virginia.
Said Home Study is attached hereto.
X.
Your Petitioners further say that they are more than fifteen (15) years older than said children sought to be adopted; that said children are not currently possessed of property of any kind or character; that your Petitioners are financially able and willing to provide said children with a proper home and to see to their education and welfare; that the minor children are not American Indian children as defined by statute; that your Petitioners, Steven Mark Lilly and Pamela Lynn Lilly are the maternal grandparents of said children and love said children as if they were their own and they are desirous of adopting said children; and that the best interests of the said children will be materially promoted by this adoption. Once this adoption is granted, it is the Petitioners’ desire to hereafter have the minor child’ s legal names changed as follows:
Madilynn Kathleen Steele shall be changed to Madilynn Kathleen Lilly.

Nathan Isaiah Steele, Jr., shall be changed to Nathan Isaiah Lilly.

WHEREFORE, Petitioners pray:
1) That, upon presentation of this Petition to the Court, the same be ordered filed with the clerk thereof;
2) That a day be appointed for a hearing on said Petition and for the examination under oath of the parties in interest;
3) That a copy of this Petition and Notice of Hearing be served upon the West Virginia Department of Health and Human Resources; and
4) That, after said hearing on the Petition so filed and the examination made, your Petitioners, Steven Mark Lilly and Pamela Lynn Lilly, be allowed to adopt said infant children, and grant unto your Petitioners such other and further relief as this Honorable Court deem proper.
