The operation is discharging into Willis Branch and Unnamed tributaries of Willis Branch; Maynor Branch of Dixons Branch; Right Fork & Unnamed Tributaries of Right Fork of Dixons Branch; all of Paint Creek; Lick Run and Unnamed Tributaries of Lick Run of Clear Fork of Big Coal River of Coal River; ALL of Kanawha River and is located 2.3 miles, southwest of Pax in Plateau, Clear Fork and Town District(s) of Fayette and Raleigh County(ies), Longitude 81o 17′ 40″ and Latitude 37o 53′ 24″ (Coordinates from USGS Topographic Map).

Written comments and/or requests for an informal conference of the permit renewal application shall identify the applicant and application number and will be received by the Permit Supervisor at the DEP address above until May 8, 2021, or thirty (30) days from the date of final publication.
