Notice is hereby given that CM ENERGY OPERATIONS LP, 200 GEORGE ST, STE 4 BECKLEY, WV 25801 has submitted an application to the Department of Environmental Protection (DEP) 1101 George Kostas Drive, Logan, WV 25601 for a significant permit revision to Article 3 permit number S400110 in order to add coal refuse disposal sites.
The revised permit will discharge into CUB CK of OHIO RV and is located 1 mile East of Coal Mountain, in the Clear Fork District of Wyoming County, Longitude 81° 42’ 54” and Latitude 37° 39’ 55” (Coordinates from USGS Topographic Map).

Written comments and/or requests for an informal conference of the surface mining application shall identify the applicant and application number and will be received by the Permit Supervisor at the DEP address above until 10/23/2025, or thirty (30) days from date of final publication.
