YOU ARE HEREBY SUMMONED AND REQUIRED to answer the Complain and Notice of Adoption Proceedings, concerning the minor child above. The above-named Court must be informed of your current address and any changes of your address during the adoption proceedings. This action has been filed in the office of the York County Family Court Clerk of Court on July 2, 2021, a copy which can be delivered to you upon request; You must serve a copy of your answer to the Complaint on the Plaintiff’s attorney, Tracy Bomar-Howze, The Howze Law Firm, 235 E. Main Street Suite 115, Rock Hill, SC 29730, within thirty (30) after service hereof upon you. If you fail to answer the Complaint within the time aforesaid or fail to appear in court, the Plaintiff in this action will apply to the court for the relief demanded in the Complaint. Failure to respond within 30 days of receipt of this Notice of Adoption Proceedings constitute your consent to the adoption and forfeiture of all of your rights and obligations to the above-named child.
