BOX 910667, LEXINGTON, KY 40591 filed an application with the Department of Environmental Protection (DEP) 1159 Nick Rahall Greenway, Fayetteville, WV 25840 for extension of inactive status on Permit No. U402199 located near SLAB FORK in SLAB FORK District of Wyoming County.
