REQUESTS FOR
QUALIFICATION STATEMENTS

The Shady Spring Public Service District of Raleigh County, West Virginia is seeking qualification statements from architects and/or engineers to provide services in regard to the Glen Morgan Wastewater Treatment Plant Upgrade. Professional services will include, but are not limited to (1) preparation of funding applications, (2) funding agency coordination, (3) preparation of design drawings and specifications for all components of the project, (4) securing all necessary permits associated with the project, (5) preparation of bidding and contracting documents, (6) participation in the evaluation of bids, and (7) monitoring and observation of construction activities.

Procurement of these services shall be in conformity with the applicable laws and regulations of the State of West Virginia, the Federal Economic Development Administration, Department of Environmental Protection, Rural Utilities Service, Appalachian Regional Commission, the Department of Housing and Urban Development, and the U.S. Army COE. Attention is directed to the fact that the proposed project may be undertaken through a variety of Federal and State funding agencies and that all services will be provided in accordance with the regulations appertaining thereto. Funding sources for the project may include but are not limited to HUD Small Cities Block Grant, Appalachian Regional Commission, USDA Rural Development, WVDEP Clean Water Sewer Revolving Fund, WV Infrastructure and Jobs Development Council, US Army COE, and local sources.

A committee of three P.S.D. Commissioners and the General Manager shall evaluate the qualification statements submitted by interested firms and select a minimum of three firms which, in their opinion, are best qualified to perform the desired services. Each of these firms will be notified of its selection for interview in order to prepare presentations.

Interviews with each selected firm shall be conducted, and based upon the interviews, the Committee shall determine the most highly qualified firm to provide the required services. The Committee shall then commence scope of services and fee negotiations with the highest-ranking firm. In the event that the Committee is unable to reach an agreement with the highest ranking firm, similar negotiations will be conducted with other firms, in descending order of ranking, until an agreement is reached in accordance with State laws and regulations.

The Shady Spring Public Service District reserves the right to accept or reject any and/or all fee proposals. The contract will be awarded to the responsible offerer whose proposal is within the competitive range and determined to be the most advantageous to the Public Service District.

The selected firm will be required to comply with the provisions of the Part VI of the Civil Rights Act of 1964, Executive Order 11246, Section 109 of the Housing and Urban Development Act of 1974, Section 3 of the Housing and Urban Development Act of 1968, Conflict of Interest Statement and Access to Records. Alternative State and Federal funding requirements may also apply.

The Shady Spring Public Service District will afford full opportunity for minority and women-owned business enterprises to submit statements of qualification in response to this invitation and will not discriminate against any firm or individual on grounds of race, creed, color, sex, age, handicap status, or national origin in the contract award
