ADVERTISEMENT FOR BIDS Southern WV Health System New Guyan Valley Wellness Center NOTICE IS HEREBY GIVEN THAT Southern WV Health System (SWVHS) (Lincoln Primary Care Center Inc.) will accept sealed bids for the construction of a new Guyan Valley Wellness Center in Branchland, WV. SWVHS Guyan Valley Wellness Center, Branchland, WV. This project consists of a new community health clinic with multiple providers’ offices, exam rooms, waiting areas, office and conference space, lab area and a pharmacy with a small retail area and drive-thru. There are also utility service areas, restrooms and employee areas in the building. The new building will be approximately 7,551 sf and will be slab-on-grade construction with some steel structural members, wood-frame construction with wood low slope roof trusses bearing on steel beams and columns and load bearing walls. The low-sloped roof will drain to interior drains and support roof mounted HVAC equipment with a membrane roof system with tapered insulation. The building exterior will be clad with masonry veneer and metal panels with aluminum storefront doors and windows. Sitework will include all grading, site utilities (water, sewer, electrical, etc.) and pavement (asphalt oncrete) for parking, drive areas and sidewalks. Demolition and site clean-up of the existing clinic building on-site will also be included as part of the work. Sealed bids will be received by the Southern WV Health System, at 7400 Lynn Avenue, Hamlin, WV 25523 until 2:00 P.M., on Thursday, August 4th. Bids received after that time will be returned to the bidder unopened. All sealed bids will be publicly opened and read aloud immediately following the scheduled closing time for receipt of bids. Interested parties are invited to attend. The Work includes required sitework, new building construction and demolition of the existing building as documented in the drawings and specifications prepared by E.T. Boggess Architect, Inc. All bids must be enclosed in sealed envelopes bearing the name of the project being bid, i.e. – “Guyan Valley Wellness Center”. Faxed bids will not be accepted. Bids may be mailed or delivered to Lisa King Leach, CEO, Lincoln Primary Care, 7400 Lynn Avenue, Hamlin, WV 25523, if received prior to bid date. This project is supported by the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services (HHS) as part of an award totaling $717,987 with a percentage financed with non-governmental sources to be determined. Each bidder will be required to furnish a certified check or bank draft payable to the Southern WV Health System or a Bid Bond executed by the Bidder and a grantee Bidder and a Bidder surety company approved by the Southern WV Health System, in an amount equal to five percent (5%) of the total projected cost of the work, materials and labor, plus proposed fee, which shall be submitted with each Bid to guarantee the Bidder’s entrance into a Contract, if given the award. No Bid shall be waived or returned because the Bidder failed to or cannot comply with any requirements as set forth in the Plans, Specifications, or any applicable statutes of the United States, the State of West Virginia and r local ordinances. A non-mandatory, walk-thru conference will be held for the Guyan Valley Wellness Center at 5233 McClellan Avenue, Branchland, WV on Tuesday, July 19th at 1:00 p.m. All general contractors bidding the project are encouraged to attend to familiarize themselves with the project location, site conditions and other relevant information. The Owner reserves the right to reject any or all bids and to waive any informalities therein. No bid may be withdrawn for a period of sixty (60) days after the time of opening of the bids. All bidders will be required to certify, indicate and acknowledge that he he has all appropriate licenses and meets all qualifications required by the laws, regulations, and ordinances of the State of West Virginia and local jurisdictions.
