ADVERTISEMENT FOR BIDS COMMUNITY CARE OF WEST VIRGINIA BRAXTON COUNTY, WEST VIRGINIA COMMUNITY CARE FLATWOODS FITOUT General Notice Community Care of West Virginia (Owner) is requesting Bids for the construction of the following Project: Community Care – Flatwoods Thrasher Project #060-10268 Bids for the construction of the Project will be received at Community Care Flatwoods located at Flatwoods Factory Outlets, 250 Skidmore Ln, Sutton, WV 26601 on Tuesday December 6, 2022 at 2:00pm local time. Bids received will be publicly opened and read. The Project includes the following Work: The renovation of 2 retail bays and the fit-out of 2 retail bays in the Flatwoods Factory Stores. From left to right along the Flatwoods Factory Outlet retail bays: Retail Bay #1, the existing Community Care, is to be renovated to include a revised layout of spaces, new finishes throughout, and extensions of existing MEP systems. Retail Bay #2 is to be fit out with a new lobby, lab, restrooms, and personnel spaces. This includes all new finishes and MEP systems. Retail Bay #3, existing Urgent Care Xpress, is to be renovated for addition personnel spaces and a pharmacy which includes new finishes and revised mechanical and electrical systems. Retail Bay #4 is to be fit out with new exam rooms and personnel spaces. This includes all new finishes and MEP systems. The total occupied space of the renovation it-out is 14,225 sf. Renovation work includes a partial demolition of exiting walls, HVAC where need to reroute xtend existing ductwork, ceiling grid and flooring as required for new layout, and concrete as required for extension of sanitary lines. The new construction in the space includes the construction of wood or metal stud walls (depending on retail bay) with resilient channels, acoustical ceilings, lighting, doors, casework, and finishes. It also includes the installation of new HVAC rooftop units associated ductwork and connections to existing ductwork. New plumbing lines to new exam rooms and restrooms and associated electrical requirements for each space. Bids are requested for the following Contract: Community Care – Flatwoods Fitout The Work will be substantially completed within 240 calendar days after the date when the Contract Times commence to run and completed and ready for final payment within 30 days after the date when the Contract Times commence to run. Liquidated damages shall be $1,000 per day. Prospective Bidders are urged to register with the designated website as a plan holder, even if Bidding Documents are obtained from a plan room or source other than the designated website in either electronic or paper format. The designated website will be updated periodically with addenda, lists of registered plan holders, reports, and other information relevant to submitting a Bid for the Project. All official notifications, addenda, and other Bidding Documents will be offered only through the designated website. Neither Owner nor Engineer will be responsible for Bidding Documents, including addenda, if any, obtained from sources other than the designated website. Envelope No. 1 must have the following information presented on the front: Name and address of Bidder Bid on Contract – Community Care Flatwoods Fitout Received by Community Care Envelope No. 2 labeled “Bid Proposal” shall also be placed inside of Envelope #1. Envelope No. 1 will be opened first, and the Bid Opening Requirement items checked for compliance as outlined on the Bid Opening Checklist on page BOR – 1 of these contract documents. If such documents are found to be in order, Envelope No. 2 “Bid Proposal”, will then be opened and will be publicly read aloud. If the documents required to be contained in Envelope No. 1 are not in order, Envelope No. 2 “Bid Proposal” will not be opened, and the Bid will be considered non-responsive and will be returned to the Bidder. A bidder may not withdraw his bid for a period of ninety (90) days after the date set for the opening of bids. Bids shall be accompanied by a certified check or bid bond payable to Community Care of West Virginia in an amount equal to five percent (5%) of the base bid. Contractor is required to provide the Drug Free Workplace Certification in accordance with WV Code 21-1D-5. All Bidders are required to have a West Virginia Contractor’s License per Section 30-42-12 of the West Virginia Code. Bids received after the scheduled closing time for the reception of bids will be returned unopened to the bidders. Community Care of West Virginia reserves the right to reject any and all bids.
