ADVERTISEMENT FOR BIDS Sealed Bids will be received by the Elkins Road Public Service District, located at 4536 Old Elkins Road Buckhannon, WV 26201, until 1:30 P.M., L.P.T., March 19, 2020 for the following work: Elkins Road Public Service District Water Distribution System Extension Phase 3 Contract No. 1: New Office Building Contract No. 2: Tank Rehabilitation Contract No. 3: Water Distribution System Extensions Contract No. 4: Water Distribution System Extensions Bid opening will be located at Kesling Mill Lions Club Building, 2138 Childers Run Road, Buckhannon, WV 26201. The total base bid price for each contract shall include the costs for furnishing labor and materials and performing all work set forth in the Advertisement for Bids, Information for Bidders, Forms of Proposal, General Conditions, Supplemental General Conditions, and Plans and Detail Specifications prepared by Chapman Technical Group. Immediately following the scheduled closing time for the reception of bids, all proposals that have been submitted in accordance with the conditions of this project will be publicly opened and read aloud. Separate Bids will be received for Contract No. 1, Contract No. 2, Contract No. 3 and Contract No. 4. The work to be bid upon is described as follows: Contract No. 1: New Office Building Contract No. 1 will consist of a Prefabricated Office Building and Garage on prepared foundation with parking area, water service and on-site sanitary sewer system, generator, and all necessary appurtenances for a complete project and is identified as Contract No. 1: New Office Building – Phase 3 Water Distribution System Extensions, as shown on contract documents prepared by Chapman Technical Group. Construction time limit is 180 calendar days for Contract No. 1. Contract No. 2: Tank Rehabilitation Contract No. 2 will consist of cleaning and re-painting the 200,000 Gallon Jack Hill Water Storage Tank and 100,000 Gallon Waughaman Water Storage Tank, including furnishing of all labor, materials, equipment, and services necessary, and is identified as Contract No. 2 – Tank Rehabilitation – Phase 3 Water Distribution System Extensions, as shown on contract documents prepared by Chapman Technical Group. Construction time limit is 270 calendar days for Contract No. 2. Contract No. 3: Water Distribution System Extensions Contract No. 3 will consist of approximately 14,600 LF of 6 – inch PVC C900, 165 LF of 6 – inch DIP RJ Creek Crossing, 225 LF 6 – inch HDPE HDD, 10,620 LF of 4-inch PVC C900, 7,670 LF of 2 – inch PVC CL 315, 6 EA Fire Hydrant Assemblies, 5 EA Blow-off Assemblies, 3 Each 6 – inch Gate Valves, 3 Each 4 – inch Gate Valves, 2 Each 2 – inch Gate Valves, 64 Each Tandem Meter Settings, 4 Each Combination Air Valves, 1 LS Constant Pressure Pumping Station, 1 LS Generator, 1 LS SCADA System, Service Laterals, and all necessary appurtenances for a complete project as identified as Contract No. 3 – Water Distribution System Extensions Phase 3 and as shown on Contract Documents prepared by Chapman Technical Group. Construction time limit is 270 calendar days for Contract No. 3. Contract No. 4: Water Distribution System Extensions Contract No. 4 will consist of approximately 31,020 LF of 6 – inch PVC C900, 72 LF of 6 – inch DIP RJ Creek Crossing, 138 LF 2″ PVC CL 315 Creek Crossing, 14,100 LF of 4-inch PVC C900, 3,920 LF of 2 – inch PVC CL 315, 20 EA Fire Hydrant Assemblies, 2 EA Blow-off Assemblies , 15 EA 6 – inch Gate Valves, 5 EA 4 – inch Gate Valves, 48 EA Tandem Meter Settings, 9 EA Combination Air Valves, 1 LS Constant Pressure Pumping Station, 6 LS Generators, Service Laterals, and all necessary appurtenances for a complete project as identified as Contract No. 4 – Water Distribution System Extensions Phase 3 and as shown on Contract Documents prepared by Chapman Technical Group. Construction time limit is 270 calendar days for Contract No. 4. Failure of any Bidder to purchase plans, specifications, and contract documents from the Engineer prior to submittal of bid will result in the rejection of said bid. These documents consist of plans, specifications and a bid submittal packet. The bid submittal packet provides an unbound copy and listing of the items which are to be submitted with the Bid. Plans and Specifications MUST be obtained by each Bidder from the office of Chapman Technical Group, 200 Sixth Avenue, St. Albans, West Virginia 25177, telephone (304) 727-5501. Payment is not refundable. Plans & Specifications: Contract No. 1: $200.00 Contract No. 2: $100.00 Contract No. 3: $250.00 Contract No. 4: $300.00 If Plans and Specifications are requested to be mailed by the Engineer, Bidders shall also pay for the cost of postage and handling. The cost of postage and handling will be $30.00 for Contracts No. 1 and No. 2 and $40.00 for Contracts No. 3 and No. 4. Each BID for each contract must be submitted in a separate sealed envelope containing the executed Form of Proposal, and included in a larger sealed envelope containing the Bid Bond, Power of Attorney, Certificate of Receipt of Addenda, Drug-Free Workplace Affidavit, WV Contractor’s License, Certification of Bidder Regarding Section 3 and Segregated Facilities, Certification of Bidder Regarding Equal Employment Opportunity, Section 3 Workforce Needs, Section 3 Project Businesses, and Section 3 Contractor’s Plan. Both sealed envelopes must have the following information in the lower left hand corner for the respective contract being bid: Contract No. 1 (as applicable) “Name and Address of Bidder, Elkins Road Public Service District, Bid on Water Distribution System Extension Phase 3, Contract No. 1: New Office Building, to be opened at 1:30 P.M., L.P.T., March 19, 2020.” The Contractor’s Qualification Statement form from the two (2) lowest bidders for each contract must be submitted to Chapman Technical Group, 200 Sixth Avenue, St. Albans, West Virginia 25177, by 4:00 P.M., L.P.T, March 26, 2020. All prospective Bidders are required to attend a mandatory Pre-Bid Conference to be held at the Kesling Mill Lions Club, located at 2138 Childers Run Road, Buckhannon, WV 26201on March 5, 2020, at 1:00 P.M., L.P.T., for the purpose of discussing the project and touring the project sites. Prospective Bidders are advised that the contracts awarded under this Advertisement for Bid is likely to be funded by a variety of federal, state, and local funds. Neither the United States nor the State of West Virginia nor any of its departments, agencies, or employees is or will be a party to this Advertisement for Bids or any resulting contract. Bidders on this work will be required to comply with the President’s Executive Order No. 11246. The requirements for bidders and contractors under this order are explained in the specifications. Bidders on this work will be required to comply with the President’s Executive Order 11246, HUD Section 109, which prohibits discrimination in employment regarding race, creed, color, sex or national origin, and have non-segregated facilities. This CDBG-Small Cities Block Grant project is to be financed in part by a grant from the United States Department of Housing and Urban Development (HUD) and the West Virginia Development Office. The work to be performed under this contract is subject to the requirements of Section 3 of the Housing and Urban Development Act of 1968, as amended, 12 U.S.C. 170l u (Section 3). The purpose of Section 3 is to ensure that employment and other economic opportunities generated by HUD assistance or HUD-assisted projects covered by Section 3, shall, to the greatest extent feasible, be directed to low- and very low-income persons, particularly persons who are recipients of HUD assistance for housing. The parties of this contract agree to comply with HUD’s regulations in 24 CFR Part 135, which implement Section 3. Bidders’ attention is directed to the Section 3 “Clause” set forth in the contract documents. Bidders must comply with the requirements for Affirmative Action and Minority Business Enterprises participation as described in the Federal Specifications insert to the Contract Documents. Bidders must comply with Title VI of the Civil Rights Act of 1964, the Davis-Bacon Act, the Copeland Act, the Contract Work Hours and Safety Act, and the West Virginia Division of Labor Wages for Construction of Public Improvements pursuant to West Virginia Code 21-5A-3. Prospective Bidders are hereby advised that it is their responsibility to adhere to all applicable Federal, State, and Local requirements and regulations including but not limited to, Equal Employment Opportunity, Segregated Facilities, Discrimination in Employment, Affirmative Action and Minority Business Participation, the Civil Rights Act of 1964, the Anti-Kickback Act, and the Contract Work Hours Standards Act. DBE REQUIREMENTS Each Bidder must fully comply with the requirements, terms, and conditions as set forth in the contract documents to achieve Disadvantaged (Minority-Owned/Women-Owned/Small) Business Enterprise (DBE) participation during the performance of this contract. The bidder commits itself to the program for DBE participation and all other requirements, terms, and conditions of the bid conditions by submitting a properly signed bid. The Bidder agrees to make “good faith efforts” to sub-contract a portion of the total value of the contract to DBE businesses. This shall be done in compliance with the six (6) affirmative steps as outlined in 40 CFR 31.36(e). Failure to demonstrate positive efforts to do so may lead to rejection of bids. For the purpose of this program, the term “subcontractor” includes all construction, modification, and service work contracted for by the bidder in the execution of the work under this contract. NONDISCRIMINATION IN EMPLOYMENT Bidders on this work will be required to comply with the President’s Executive Order No. 11246. Prospective Bidders are hereby advised that it is their responsibility to adhere to all applicable Federal, State, and Local requirements and regulations including but not limited to, Equal Employment Opportunity, Segregated Facilities, Discrimination in Employment, Affirmative Action and Minority Business Participation, the Civil Rights Act of 1964, the Anti-Kickback Act, and the Contract Work Hours Standards Act. Bids shall be accompanied by a bid bond or a certified check in an amount equal to five percent (5%) of the bid to insure the execution of the contract for which the bid is made. In case the bid is not accepted, the check or bid bond will be returned to the bidder, but if the bid is accepted and the bidder shall refuse or neglect to enter into a contract with the Elkins Road Public Service District within fifteen (15) days after the time he has been notified of the acceptance of his bid, the said check or bid bond shall be forfeited to the Elkins Road Public Service District as liquidated damages for the failure to do so. No Bidder may withdraw his Bid for a period of ninety (90) days after the time of the opening of the Bids. All bidders are required to provide a valid WV Contractor’s License Number with their bid per Section 21-11-11 of the West Virginia Code. Any Contractor submitting a Bid on this project hereby certifies, indicates, and acknowledges that he has a valid and current WV Contractor’s license, has examined and acknowledges site and working conditions, and meets all the qualifications required by the statutes of the State and subdivision in which the work is to be performed.
