ADVERTISEMENT FOR BIDS CAMDEN FAMILY HEALTH CAMDEN-ON-GAULEY, WEST VIRGINIA CAMDEN FAMILY HEALTH DENTAL CLINIC RENOVATIONS THRASHER PROJECT #060-10107 Sealed Bids for the Camden Family Health Dental Clinic Renovations will be received by Camden Family Health, located at 10003 Webster Road, Camden-On-Gauley, West Virginia 26208, until 1:30 p.m., E.S.T., on March 26, 2020 for furnishing labor and materials and per forming all Work set forth in the Contract Documents prepared by The Thrasher Group, Inc. Immediately following the scheduled closing time for the reception of Bids, all proposals which have been submitted in accordance with the conditions of the Contract Documents will be publicly opened and read aloud. The approximate quantities of Work to be Bid upon are as described as follows: The of the Project includes interior renovation of an unfinished portion of an existing outpatient health clinic. This interior renovation work includes removal of ductwork, removal of light fixtures, removal of interior gypsum board and stud wall partitions, removal of floor finishes, and removal of mechanical, electrical, and plumbing fixtures, equipment, and distribution. Once the interior demolition has taken place, construction activities shall include new gypsum and metal stud wall partitions, new lay-in acoustic tile ceilings, new floor finishes, new doors and frames, painting, and new plumbing and electrical fixtures, equipment, and distribution. The Work will be substantially completed within 180 consecutive calendar days. Final completion will be 30 days thereafter. Liquidated damages shall be $500.00 per day. O. Box 940, Bridgeport, WV 26330, for the following costs: Bidding Documents issued as hard copy drawings and hard copy specifications are available for $50 per set. Bidding Documents issued as digital drawings and digital specifications on a C.D., are available for $25 per set. Amounts paid are not subject to refund. A two envelope system will be used. Envelope No. 1 must have the following information presented on the front: Name and address of Bidder Bid for Camden Family Health Dental Clinic Renovations Received by Camden Family Health Envelope No. 2 labeled “Bid Proposal” shall also be placed inside of Envelope #1. Envelope No. 1 will be opened first and the Bid Opening Requirement items checked for compliance as outlined on the Bid Opening Checklist on page BOR – 1 of these contract documents. If such documents are found to be in order, Envelope No. 2 “Bid Proposal”, will then be opened and will be publicly read aloud. If the documents required to be contained in Envelope No. 1 are not in order, Envelope No. 2 “Bid Proposal” will not be opened and the Bid will be considered non-responsive and will be returned to the Bidder. A Bidder may not withdraw his Bid for a period of thirty (30) days after the date set for the opening of Bids. Bids shall be accompanied by a Bid Bond payable to the Camden Family Health, an amount equal to five percent (5%) of the Total Bid. Contractor is required to provide the Drug Free Workplace Certification in accordance with WV Code 21-1D-5. All Bidders are required to have a West Virginia Contractor’s License per Section 21-11-11 of the West Virginia Code. Bids received after the scheduled closing time for the reception of Bids will be returned unopened to the Bidders. Camden Family Health reserves the right to reject any and all Bids. A MANDATORY pre-bid conference will be held at the project site, located at 415 Main Street, Summersville, West Virginia 26651, on March 12, 2020, at 11:00 a.m., E.S.T.
