ADVERTISEMENT FOR BIDS Southern WV Health Systems New Wellness Center NOTICE IS HEREBY GIVEN THAT Southern WV Health Systems (SWVHS) (Lincoln Primary Care Center Inc.) will accept sealed bids for the construction of a new Wellness Center in Hamlin, WV. SWVHS Hamlin Wellness Center, Hamlin, WV. This project consists of the building construction and sitework required for a new 6,070 sf, slab-on-grade pre-engineered metal building structure to accommodate a community multi-purpose area, served by a commercial kitchen, restrooms, and utility/storage areas. The building will also accommodate an exercise/wellness area that will also be served by restrooms, utility and storage areas. The metal building will include metal panels, metal roof system and some masonry accents. There will be aluminum storefront doors and windows. Work to include complete mechanical (HVAC & plumbing) and electrical systems for the building and site. Sitework will include, but is not limited to, grading, site paving (asphalt and concrete), all site utilities to tie into existing systems (water, sewer, electrical, etc. and site lighting. Sealed bids will be received by the Southern WV Health Systems, at 7400 Lynn Avenue, Hamlin, WV 25523 until 2:00 P.M., on Wednesday, December 22nd. Bids received after that time will be returned to the bidder unopened. All sealed bids will be publicly opened and read aloud immediately following the scheduled closing time for receipt of bids. Interested parties are invited to attend. The Work includes renovations to the existing building as documented in the drawings and specifications prepared by E.T. Boggess Architect, Inc. All bids must be enclosed in separate, sealed envelopes bearing the name of the project being bid, i.e. “Hamlin Wellness Center”. Faxed bids will not be accepted. Bids may be mailed or delivered to Lisa King Leach, CEO, Lincoln Primary Care, 7400 Lynn Avenue, Hamlin, WV 25523, if received prior to bid date. Bidders on this work may not discriminate in employment with regards to race, creed, color, sex, or national origin. Bidders must comply with the requirements of all applicable state and federal laws. Bidders must also comply with West Virginia Code 21-1D-5 requiring a Drug Free Workplace. Each bidder will be required to furnish a certified check or bank draft payable to the Southern WV Health Systems or a Bid Bond executed by the Bidder and a grantee Bidder and a Bidder surety company approved by the Southern WV Health Systems, in an amount equal to five percent (5%) of the total projected cost of the work, materials and labor, plus proposed fee, which shall be submitted with each Bid to guarantee the Bidder’s entrance into a Contract, if given the award. No Bid shall be waived or returned because the Bidder failed to or cannot comply with any requirements as set forth in the Plans, Specifications, or any applicable statutes of the United States, the State of West Virginia and/or local ordinances. The cost for each set of printed bid documents is $250 and is nonrefundable. The cost for a CD and/or a download link is $50 and is nonrefundable. A non-mandatory, walk-thru conference will be held for Hamlin Wellness Center at the site, located beside the LCHS Panther Center, 81 Lincoln Panther Way, Hamlin, WV on Wednesday, December 8h at 1:00 p.m. All general contractors bidding the project are encouraged to attend to familiarize themselves with the project location, site conditions and other relevant information. The Owner reserves the right to reject any or all bids and to waive any informalities therein. No bid may be withdrawn for a period of sixty (60) days after the time of opening of the bids. All bidders will be required to certify, indicate and acknowledge that he/she has all appropriate licenses and meets all qualifications required by the laws, regulations, and ordinances of the State of West Virginia and local jurisdictions.
