NOTICE REQUEST FOR PROFESSIONAL SERVICES The Cabell County Community Services Organization (CCCSO) requests letters of interest from qualified professional firms for the design of the West End Senior Wellness Center. The successful firm will be contracted to prepare plans, contracts, specifications, and construction oversight for the proposed 2,000 square foot facility. The project also includes site work and the development of outdoor congregate space Services will begin on or about February, 2022 with final plans complete by May 1, 2022. Proposals will be received until 3:00 p.m., local prevailing time on January 28, 2022, at CCCSO, located at 724 10th Avenue Huntington, WV. Each proposal must include the following: 1. Firm name, address, phone number, fax number 2. Names and resumes listing the technical qualification of the persons who will perform these services. 3. Description of experience in related work. 4. Reference list of clients whom the entity has represented in similar projects, with names and current telephone numbers of contacts. 5. Five copies of the proposals are to be submitted. The following criterion will be used in the selection process: 1. Project Manager. (20 points max) 2. Strength/Experience of Assigned Staff including Subconsultants. (15 points max) 3. Familiarity with the project. (30 points max) 4. Location (10 points max) 5.
