MONITORING REQUIREMENTS NOT MET FOR

We are required to monitor your drinking water on a regular basis. Results of regular monitoring are an Indicator of whether or not our drinking water meets health standards. Between 1/1/2021 and 12/31/2021, we failed to submit the required samples as indicated below and therefore cannot be sure of the quality of our drinking water during that time.

What happened? What is being done?
Wrong address was added to the Chain of Custody form. Sample was taken at the right location but stated wrong address.
Corrections have been made to assure this mistake will not happen again. All files, names and addresses have been updated and thoroughly checked to catch any mistakes on all paperwork.
New procedures and protocols are in place to ensure proper communications and result submittals.
