England, Family Court Judge, Family Court of Fayette County, West Virginia, in the Office of the Family Court Judge located at 213 North Court Street, Fayetteville, Fayette County, West Virginia, or as soon thereafter as this case may be heard, and request a name change to clarify his name to be reflected on his birth certificate as provided by statutory law of the State of West Virginia.
