You will please take notice that the undersigned 84 Lumber Company, L.P., 1019 Route 519 – Bldg. #3, Eighty Four, PA 15330, Phone: (724) 228-8820, Fax: (877) 341-2753 has furnished and delivered to Legacy Building and Design, Inc., 143 N MAIN ST, KERNERSVILLE, NC 27284-2899 who was contractor with you (or subcontractor with Legacy Building and Design, Inc., 143 N MAIN ST, KERNERSVILLE, NC 27284-2899, who was contractor with you, as the case may be) for use in the erection and construction (or repair, removal, improvement, as the case may be) of on the real estate known as 3841 FLINT HOLLOW RD, Clintonville, WV 24931 in the County of Greenbrier, APN: District 03 Map 17 Parcel 8, and the said materials were of the nature and were furnished on the dates and in the quantities and at the price as shown in the following account thereof: Building materials, related materials and/or labor, our Job/Invoice #: 04216601.
You are further notified that the undersigned has not been paid the sum of $1,972.88 (or that there is still due and owing to the undersigned thereon the sum of $1,972.88 and that he claims a lien upon your interest in the said lot (or tract) of land and upon the buildings, structures and improvements thereon, to secure the payment of the said sum.
