NOTICE OF APPLICATION FOR CONTRACT CARRIER AUTHORITY AND HEARING On November 12, 2015, the Applicant, EnAct, Inc., filed an application for a permit to operate as a contract carrier to transport passengers in non-emergency medical transport under contract with Medical Transportation Management, Inc. (MTM). According to EnAct, it seeks to transport Medicaid passengers to appointments and be reimbursed by Medicaid through the West Virginia Department of Health and Human Resources and MTM. On December 29, 2015, the Applicant clarified its request to indicate it seeks to pick up passengers located in Kanawha, Fayette, Boone, Putnam and Jackson Counties and transport said passengers to and from doctor appointments statewide. The proposed certificate will provide that the Applicant has permission: To operate as a contract carrier by motor vehicles in the transportation of passengers for non-emergency medical transport in Kanawha, Fayette, Boone, Putnam and Jackson Counties, West Virginia. Rates: Curb to Curb Loaded Business Hours Rates per One-Way Trip Mileage Band: 0.00-10:00 Miles Ambulatory $13.00/Base/Wheelchair $21.00/Base 10.01-Endless Miles Ambulatory $1.25/Mile/Wheelchair $1.50/Mile Additional Services and Fees Wait time: Time a driver is stationed without any pickups after drop off of 40 miles Loaded from Provider’s facility $0.33 per minute. Unloaded Mileage: Mileage rate per one-way trip after 15 miles from Provider’s main facility or sub-station: $0.90 per mile. On January 15, 2016, the matter was set for hearing on April 13, 2016, at 10:00 a.m. in the Public Service Commission’s Howard M. Cunningham Hearing Room, 201 Brooks Street, Charleston, West Virginia.
