NOTICE OF CUSTOMER MEETING

OTSEGO COMMUNITY WATER SYSTEM NOTICE OF CUSTOMER MEETING October 15, 2015, 7:00 p.m. at the Otsego Bible Chapel Transfer of Otsego Community Water System Customers to Eastern Wyoming Public Service District The Officers of the Otsego Community Water System (“Water System”) have determined that it is in the best interest of the customers of the Water System to transfer the customers to Eastern Wyoming Public Service District. It has become impossible for the Water System to effectively operate, and to meet the requirements of the West Virginia Bureau for Public Health. A community meeting for customers of the Water System will be held on Thursday, October 15, 2015 at 7:00 p.m. at the Otsego Bible Chapel Community Room, Otsego, WV to provide additional information concerning the transfer. Representatives of the Eastern Wyoming PSD will be in attendance to answer any questions you may have. A petition has been filed with the Public Service Commission of West Virginia requesting approval for this transfer. Upon the effective date of the transfer, which is expected to be in December, 2016, Eastern Wyoming PSD will charge its then-current system wide water rates that have been previously approved by the Public Service Commission which are: RATES First 2,000 gallons used per month $11.33 per 1,000 gallons Next 58,000 gallons used per month $10.84 per 1,000 gallons Next 240,000 gallons used per month $7.96 per 1,000 gallons All Over 300,000 gallons used per month $4.09 per 1,000 gallons MINIMUM CHARGE No minimum bill will be rendered for less than the following amounts, according to the size of the meter installed. 5/8 inch meter $22.66 per month 3/4 inch meter $33.99 per month 1 inch meter $56.65 per month 1 – 1/4 inch meter $82.71 per month 1 – 1/2 inch meter $113.30 per month 2 inch meter $181.28 per month 3 inch meter $362.56 per month 4 inch meter $566.50 per month 6 inch meter $1,133.00 per month 8 inch meter $1,812.80 per month FLAT RATE CHARGE (Customer with non-metered water supply) Equivalent to 4,500 gallons of water usage $49.76 per month. DELAYED PAYMENT PENALTY The above schedule is net. On all accounts not paid in full when due, ten percent will be added to the net current amount unpaid. This delayed payment penalty is not interest and is to be collected only once for each bill where it is appropriate. TAP FEE The following charges are to be made whenever the utility installs a new tap to serve an applicant. A tap fee of $100.00 will be charged to customers applying for service before construction is completed adjacent to the customer’s premises in connection with a certificate proceeding before the Commission. This pre-construction tap fee will be invalid after the completion of construction adjacent to an applicant’s premises that is associated with a certificate proceeding. A tap fee of $300.00 will be charged to all customers who apply for service outside of a certificate proceeding before the Commission for each new tap to the system. RECONNECTION CHARGE – $20.00 To be charged whenever the supply of water is turned off for violations of rules, non-payment of bills, or fraudulent use of water. RETURNED CHECK CHARGE A service charge equal to the actual bank fee assessed to the District or a maximum of $25.00 will be imposed upon any customer whose check for payment of charges is returned by their bank due to insufficient funds. LEAK ADJUSTMENT $1.10 per 1,000 gallons is to be used when a bill reflects unusual water consumption which can be attributed to eligible leakage on customer’s side of the meter. This rate shall be applied to all such consumption above customer’s historical average usage. SECURITY DEPOSIT A deposit of $50.00 or 2/12 of the average annual usage, whichever is greater, based on the applicant’s specific customer class. MUNICIPAL EXCISE TAX SURCHARGE The municipalities listed below having imposed public utility tax computed on the basis of two percent of the revenues from water sales by the Eastern Wyoming Public Service District water utility within the corporate limits of such municipalities, said tax shall be billed as a “surcharge” to the customers receiving service within said corporate limits. The water utility is required to collect the utility tax pursuant to West Virginia Code c8-13-5a. Customers receiving water service within the corporate limits of the specified municipality shall pay a surcharge based on the following rates: City of Mullens 2{4ceb532c6f579389df471c6c1e832caf2346b74dc60fcbf6aabd4d29df3baf9c} SURCHARGE $2.57 per bill per month for a period not to exceed 10 years or until the amount is paid in full from the effective date of this tariff will be charged in order to eliminate the arrearage to the Logan County Public Service District.
