PUBLIC SERVICE COMMISSION OF WEST VIRGINIA CHARLESTON CASE NO. 16-1530-PWD-CN DANESE PUBLIC SERVICE DISTRICT Application for a certificate of convenience and necessity to certain additions and improvements to its existing waterworks system. NOTICE OF FILING On November 9, 2016, Danese Public Service District (District) filed a duly verified application for a certificate of convenience and necessity to construct certain additions improvements to its existing waterworks system. The District proposes to construct certain additions and improvements to its waterworks system consisting of: (i) installation of approximately 37,700 linear feet of six-inch water 1ine, 28,500 linear feet of four-inch water line and 20,500 linear feet of two-inch water line; (ii) installation of one two-inch PRV station; (iii) installation of one 88,000 gallon water storage tank; (iv) installation of one 116 gpm booster station and (v) installation of all necessary mechanical, electrical, piping; valves, controls and appurtenances related thereto. The project will allow District to provide new water service to approximately 43 new customers in the Highland Mountain and Crickmer Road area of Fayette County. Included in this scope of work is the complete replacement of the water system serving the Chestnut Knob pressure zone. The application is on file with and available for public inspection at the Public Commission, 201 Brooks Street, in Charleston, West Virginia. The estimated cost of the project is $4,469,162. The District proposes to fund the project with a $4,469,162 grant from the Abandoned Mine Lands Program of the West Virginia Department of Environmental Protection. No rate increase is required for the project. The District will continue to charge its existing rates to its existing and new customers as follows: APPLICABILITY Applicable within the entire territory served. AVAILABILITY Available for general domestic, commercial, industrial service and resale water service RATES (customers with metered water supply) First 3,000 gallons used per month $13.99 per 1,000 gallons Next 7,000 gallons used per month $11.30 per 1,000 gallons Next 10,000 gallons used per month $9.55 per 1,000 gallons All over 20,000 gallons used per month $8.78 per 1,000 gallons MINIMUM CHARGE No bill will be rendered for less than the following based on meter size: 5/8 inch meter – $41.97 per month 3/4 inch meter – $62.96 per month 1 inch meter – $104.93 per month 1- 1/4 inch meter – $153.19 per month 1-1/2 inch meter – $209.85 per month 2 inch meter – $335.76 per month 3 inch meter – $671.52 per month 4 inch meter – $1,049.25 per month 6 inch meter – $2,098.50 per month 8 inch meter – $3,357.60 per month DELAYED PAYMENT PENALTY The above schedule is net. On all accounts not paid in full when due, ten percent (10{4ceb532c6f579389df471c6c1e832caf2346b74dc60fcbf6aabd4d29df3baf9c}) will be added to the net current amount unpaid. This delayed payment penalty is not interest and is to be collected only once for each bill where it is appropriate. TAP FEE The following charges are to be made whenever the utility installs a new tap to serve an applicant: A tap fee of $350.00 will be charged to all customers applying for service outside o certificate proceeding before the Commission for each new tap to the system. RECONNECTION FEE – $25.00 To be charged whenever the supply of water is turned off for violations of rules, nonpayment of bills or fraudulent use of water. RETURNED CHECK CHARGE A service charge equal to the actual bank fee assessed to the District up to a maximum o twenty-five dollars ($25.00) will be imposed upon any customer whose check for charges is returned by the bank due to insufficient funds. LEAK ADJUSTMENT RATE $1.68 per 1,000 gallons is to be used when a bill reflects unusual consumption which can be attributed to eligible leakage on the customer’s side of the meter. This rate shall be applied to all such consumption above the customer’s historical average usage. SECURITY DEPOSIT Not to exceed two-twelfths (2/12) of the average annual usage of the applicant’s spe fic customer class, or fifty dollars ($50.00), whichever is greater. This fee may change by applicable statutory provisions. The District does not have any resale customers. Anyone desiring to protest or intervene should file a written protest or request to intervene within thirty (30) days following the date of this publication unless otherwise modified by Commission order. Failure to timely protest or intervene can affect your right to protest aspects of this certificate case or participate in future proceedings. All protests or requests to intervene should briefly state the reason for the protest or request to intervene. Requests to intervene must comply with the rules on intervention set forth in the Commission Rules of Practice and Procedure.
