IN THE CIRCUIT COURT OF FAYETTE COUNTY, WEST VIRGINIA IN RE: The Adoption of J.C.W. by Petitioners, L.H. and J.S.H, and Petition to change his name from J.C.W to J.C.H. Civil Action No.: 15-A-9 The Honorable Judge Paul M. Blake, Jr. NOTICE Please be advised that the Petitioners will appear before the Honorable Paul M. Blake, Jr., Judge of the Circuit Court of Fayette County, West Virginia, in his Courtroom at the Fayette County Judicial Annex, on July 13, 2015, at 10:30 a.m. for a Hearing on the Petition for the Adoption of J.C.W., an infant. Further, the Petitioners are attempting to locate Chris Burkett, who based upon information and belief, may be the biological father to the infant. The whereabouts of Chris Burkett are unknown. If Chris Burkett wishes to appear in this matter, then he must file an appropriate answer to the Petition within 30 days of May 25, 2015. He may obtain a copy of the pleadings from the Fayette County Circuit Clerk’s Office at the Fayette County Courthouse, Fayette County, West Virginia. Failure to file an appropriate answer to this Petition may result in the termination of his parental rights, if any. If he fails to file an appropriate answer to this Petition within 30 days of May 25, 2015, then he may not appear in or receive any further notice of the adoption proceedings.
