NOTICE OF OPPORTUNITY FOR HEARING
In accordance with Chapter 119 and Title 39 of the Ohio Revised Code, William Seelbach (“Seelbach”) NPN 2214112, is hereby notified that the Superintendent of Insurance intends to suspend, revoke or refuse to renew his license as an insurance agent in the State of Ohio and/or take any other action or actions authorized pursuant to section 3905.14(D) of the Ohio Revised Code including civil penalties and/or administrative costs. A copy of the Notice may be obtained from Ohio Department of Insurance website (www.ohio.insurance.gov) or Darcy Moulin, Ohio Department of Insurance, 50 W. Town St., 3rd Floor, Suite 300, Columbus, Ohio 43215.
Seelbach is entitled to a hearing if he requests one on or before January 10, 2020. Such request should be addressed to Elizabeth Chase, Program Administrator 2, Ohio Department of Insurance, 50 W. Town St., 3rd Floor, Suite 300, Columbus, Ohio 43215. At the hearing, Seelbach may appear in person, by his attorney, or by such other representative as is permitted to practice before the agency, or he may present his position, arguments or contentions in writing and at a hearing he may present evidence and examine witnesses appearing for and against him.
