The object of the above entitled action is for Chad Stradwick to legally change his/her name to Chad Stradwick Sr. Notice is hereby given that the undersigned will apply by petition to the Circuit Court of Ohio County, West Virginia, for the entering of an Order by said Court for changing the name of Chad Stradwick to Chad Stradwick Sr.
A hearing shall be on the 7th day of June, 2022 at 12:30PM o’clock before the Honorable Judge Wilson at the Ohio County Courthouse. Anyone interested may appear to protect their interest. The hearing may be rescheduled without further notice or publication.
