IN THE MATTER OF OHIO COUNTY CASE # 15-CJA-107/108; REBECCA BLAKE, biological mother of infants W.G., whose date of birth is November 21,2009 and H.G., whose date of birth is April 14, 2011. Natural children of REBECCA BLAKE.
The Petitioner, the West Virginia Department of Health and Human Resources has filed a Petition that may result in the termination of parental rights of REBECCA BLAKE, biological mother of infants W.G., whose date of birth is November 21,2009 and H.G., whose date of birth is April 14, 2011.
The Petitioner alleges that REBECCA BLAKE, biological mother of infants W.G., whose date of birth is November 21,2009 and H.G., whose date of birth is April 14, 2011 has abandoned and /or failed and neglected to provide for the care, custody and control of infants W.G., whose date of birth is November 21, 2009 and W.G., whose date of birth is April 14, 2011 and has made no effort to provide for said care of infants, or maintain the infants in any way from that time until present. It is ORDERED that, REBECCA BLAKE, biological mother of infants W.G., whose date of birth is November 21,2009 and H.G., whose date of birth is April 14, 2011 is notified that she may obtain a copy of the Petition from the Clerk of the Circuit Court of Ohio County, West Virginia and each is further ORDERED to appear in the office of the Honorable James P. Mazzone, Judge of the Circuit Court of Ohio County, West Virginia, Fifth Floor, Ohio County Courthouse, Wheeling, West Virginia on the 4th day of August, 2016 at 10:45 a.m. for a Disposition Hearing if they so desire to object or make other defense to the petition, otherwise, REBECCA BLAKE, biological mother of infants W.G., whose date of birth is November 21,2009 and H.G., whose date of birth is April 14, 2011, may be terminated.
