You will please take notice that the undersigned, Carpet Showcase Flooring Center, LLC, 3527 Jacob St., Wheeling, WV, 26003, a West Virginia Limited Liability Company, was and is subcontractor with GL Housing Construction, LLC, 1020 Huron Road, Suite 100, Cleveland, Ohio, 44115, and/or Great Lakes Financial Group, 1020 Huron Road, Suite 100, Cleveland, Ohio, 44115, and/or J&S Management, 1020 Huron Road, Suite 100, Cleveland, Ohio, 44115 who were and are general contractors for the furnishing of materials and doing of the work and labor necessary to the completion of the improvements, renovation, repairing, removing, replacing, refurnishing, and/or rehabilitating carpet, flooring and trim on that certain building owned by Arbors of Windsor Manor, LLC, known as Windsor Manor, 1143 Main St., Wheeling, West Virginia, 26003, District 10-Wheeling, Map W50B, Parcel 0035, SQ 1 PTS 3-4-7-8 & PTS 7-8 9124 SQ FT OR .21 AC, as more fully described in the deed attached as Exhibit #1, and that the contract price and value of said work and materials is Twenty-One Thousand Nine Hundred Twenty- Four Dollars and Seventy-Four Cents ($21,924.74).
Carpet Showcase Flooring Center, LLC last furnished labor and materials to this property on August 12, 2016. A copy of invoices itemizing the date, price and the labor and materials provided to the buildings located on the Windsor Manor property are attached at Exhibit #2.
Arbors of Windsor Manor, LLC, is further notified that the undersigned has not been paid therefor and that Carpet Showcase Flooring Center, LLC claims and will claim a lien upon your interest, the Windsor Manor, 1143 Main St., Wheeling, West Virginia, 26003, District 10-Wheeling, Map W50B, Parcel 0035, SQ 1 PTS 3-4-7-8 & PTS 7-8 9124 SQ FT OR .21 AC, in the said lot (or tract) of land, as more fully described in the deed attached as Exhibit #1, and upon the buildings, structures and improvements thereon to secure the payment of the said sum.
