TO: The Unknown Fathers of the above referenced children, and
Any other individual claiming parental rights to the

referenced children.
The object of the above styled civil action is for the
Petitioner, West Virginia Department of Health and
Human Resources, to obtain a judicial decree that you have abandoned your children and seek to permanently terminate your parental rights.
It appear by Affidavit filed in this action that the
biological mother of the above referenced children is Konnie Gorby, the putative father of the above
referenced children is Justin Blake or the fathers of the above referenced children are unknown..
It is hereby ORDERED that the Respondents appear at the
proceedings scheduled for this matter to be held on the 16th day of January, 2024, at 1:30 P.M., in the
Courtroom at the Wetzel. County
2023. Courthouse, Main Street, New Martinsville, West Virginia, to protect your own interest, otherwise an
Order may be entered permanently terminating your parental rights. A copy of said Petition may be obtained from the undersigned Clerk at the Clerk’s Office, and in the event you are unable to afford legal counsel,
counsel will be appointed for you upon your application.
