LEGAL NOTICE
This Notice of Dissolution (“Notice”) is given pursuant to Ohio Code Section 17-1706 that Wheeling Pediatrics LLC (the “Company”) has formally dissolved by filing Articles of Termination with the Ohio Secretary of State on March 4, 2024. Any person having a claim against the Company is hereby requested to present such claim in writing to Wheeling Pediatrics LLC c/o Chief Legal Officer, West Virginia United Health System, Inc., 1085 Van Voorhis Road, Suite 500, Morgantown, West Virginia 26505, setting forth the name, address and phone number of the claimant and clearly stating the type of claim and the basis upon which the claim is made. A claim against the Company is barred unless a proceeding to enforce the claim is commenced within two (2) years after publication of this Notice
