IN THE MATTER OF OHIO COUNTY CASE # 16-CJA-108 KAYSHA KNOX, biological mother and primary custodian of infant L.C., whose date of birth is May 10, 2016 and JASON COOK, biological father of infant L.C., whose date of birth is May 10, 2016. Natural child of KAYSHA KNOX.
The Petitioner, the West Virginia Department of Health and Human Resources has filed a Petition that may result in the termination of parental rights of KAYSHA KNOX, biological mother and primary custodian of infant L.C., whose date of birth is May 10, 2016 and JASON COOK, biological father of infant L.C., whose date of birth is May 10, 2016.
The Petitioner has filed a Petition alleging that KAYSHA KNOX, biological mother and primary custodian of infant L.C., whose date of birth is May 10, 2016 and JASON COOK, biological father of infant L.C., whose date of birth is May 10, 2016, have abused and neglected, abandoned, or failed to provide for the care, custody and control of infant L.C., whose date of birth is May 10, 2016. It is ORDERED that KAYSHA KNOX and JASON COOK are notified that they may obtain a copy of the Petition from the Clerk of the Circuit Court of Ohio County, West Virginia and they are further ORDERED to appear in the courtroom of the Honorable James P. Mazzone, Judge of the Circuit Court of Ohio County, West Virginia, Fifth Floor, Ohio County Courthouse, Wheeling, West Virginia on the 13th day of January, 2017 at 1:30 p.m. for a Disposition Hearing if they so desire to object or make other defense to the Petition; otherwise, parental rights of KAYSHA KNOX, biological mother and primary custodian of infant L.C., whose date of birth is May 10, 2016 and JASON COOK, biological father of infant L.C., whose date of birth is May 10, 2016, may be terminated.
