The object of the above entitled action is for Leah Grace Daily to legally change her name to Leah Grace Fazzolari. Notice is hereby given that the undersigned will apply by petition to the Circuit Court of Ohio County, West Virginia, for the entering of an Order by said Court for changing the name of Leah Grace Daily to Leah Grace Fazzolari.
A hearing shall be on the15th day of April, 2024 at 9:30 am o’clock before the Honorable Judge Sims at the Ohio County Courthouse. Anyone interested may appear to protect their interest. The hearing may be rescheduled without further notice or publication.
