otherwise claiming parental rights to the
above referenced children.
The purpose of the above styled civil proceedings is for the West Virginia Department of Human Services to
obtain a judicial decree finding that you have abused, neglected and/or abandoned the children, D.L., born on
September 24, 2024, and E. L-H, born on December 6, 2023. These proceedings may
result in the permanent termination of your
parental rights to the children.
It appearing by an Affidavit filed in this matter that
Samantha Logan is the mother of the children, that Cane Hartwick is the named putative father of the
children and that there may be other unknown
individuals who have or claims to have parental rights to the children.
You are hereby ORDERED to appear in these
proceedings at a hearing scheduled on the 18th day of December, 2024, at 10:00 o’clock a.m., in the courtroom located at the Wetzel County Courthouse, 200 Main Street, New Martinsville, West Virginia, to protect and defend your interests. If you do not appear, an Order may be entered permanently terminating your parental rights to D.L. and E.L-H. A copy of the Petition to
Institute Child Abuse and Neglect Proceedings and the Notice of Termination of Parental Rights may be
obtained at the Wetzel County Circuit Clerk’s Office at 200 Main Street, New Martinsville, West Virginia. In the event you are unable to afford legal counsel to protect your interests, counsel will be appointed for you upon the completion of an application available in the Clerk’s Office.
