horizontal well, Incline
Unit 1H , located at
approximately UTM NAD 83 Easting 500575.518m, UTM NAD 83 Northing 4370502.072m in Outlet Middle Island Creek
Watershed, Middlebourne Quadrangle, Union
District, Tyler County, WV which disturbs three acres or more of surface
excluding pipelines,
gathering lines and roads or utilizes more than two hundred ten thousand
gallons of water in any thirty day period. To

copies or scans of the
proposed permit
application will cost
$15.00, whether mailed or obtained at DEP
