Unit 1H , located at
approximately UTM NAD 83 Easting 500567.495m, UTM NAD 83 Northing
4370497.693m in Outlet Middle Island Creek
Watershed, Middlebourne Quadrangle, Union
District, Tyler County, WV which disturbs three acres or more of surface
excluding pipelines,
gathering lines and roads or utilizes more than two hundred ten thousand

comments must reference the county, well number, and operator and be

copies or scans of the
proposed permit
application will cost
$15.00, whether mailed or obtained at DEP
headquarters. For
