proximately UTM NAD 83 Easting 500571.507m,
UTM NAD 83 Northing 4370499.882m in Outlet Middle Island Creek
Watershed, Middlebourne Quadrangle, Union
District, Tyler County, WV which disturbs three acres or more of surface
excluding pipelines,
gathering lines and roads or utilizes more than two hundred ten thousand
gallons of water in any
thirty day period. To
