The object of the above entitled action is for Jase Michael Timberlake to legally change his/her name to Jase Michael Collins. Notice is hereby given that the undersigned will apply by petition to the Circuit Court of Ohio County, West Virginia, for the entering of an Order by said Court for changing the name of Jase Michael Timberlake to Jase Michael Collins.
A hearing shall be on the 5th day of August, 2019 at 11:00am o’clock before the Honorable Judge Ronald Wilson at the Ohio County Courthouse. Anyone interested may appear to protect their interest. The hearing may be rescheduled without further notice or publication.
