TO: VICTORIA A. FLORES, MOTHER OF A.B., AND ANY AND ALL WHO CLAIM PARENTAL AND/OR CUSTODIAL RIGHTS IN AND TO SAID INFANT, A.B.
YOU ARE HEREBY NOTIFIED THAT:
1. A Petition has been filed in the above styled action alleging that the said infant is neglected and/or abused pursuant to Chapter 49, Article 1, Section 201, of the West Virginia Code, as amended; seeking placement of the temporary and permanent custody of the said infant in the West Virginia Department of Health and Human Resources; and that appropriate disposition with reference to the custody, care, control and guardianship of said infant child be made pursuant to Chapter 49, Article 4, Section 604, of the West Virginia Code, as amended.
2. Said infant has a right to counsel at every stage of the proceedings.
3. You have the right to counsel at every stage of the proceedings. If you are unable to afford to pay for your own counsel, the Court will appoint a competent attorney to represent and protect your interests..
4. Proceedings under this Petition can result in the permanent termination of your parental and/or custodial rights.
5. YOU ARE HEREBY NOTIFIED THAT A HEARING IN THE ABOVE STYLED ACTION IS SCHEDULED IN THE CIRCUIT COURT OF LEWIS COUNTY, WEST VIRGINIA, ON THE 6th DAY OF NOVEMBER, 2019, AT THE HOUR OF 11:00 O’CLOCK A.M., IN THE CIRCUIT COURTROOM OF THE LEWIS COUNTY JUDICIAL ANNEX, 117 COURT AVENUE, SITUATE IN THE CITY OF WESTON, LEWIS COUNTY, WEST VIRGINIA, AT WHICH TIME AND PLACE YOU MAY APPEAR AND SHOW CAUSE AGAINST THE SAME IF YOU SO DESIRE.
6. A copy of said Petition and Orders may be obtained from the Clerk of Circuit Court of Lewis County, West Virginia, Lewis County Judicial Annex, 117 Court Avenue, Weston, West Virginia 26452.
