REQUEST FOR BID PROPOSAL
New EMS Station – Middlebourne, West Virginia

1. INTRODUCTION
The Tyler County Commission, in partnership with
Tyler County EMS, is soliciting sealed bids from
qualified contractors for the design, delivery, and
construction of a Turn-Key Modular EMS Station to be
located in Middlebourne, West Virginia.

The proposed structure will include a modular
residential-style living quarters and an attached
apparatus/vehicle garage. All bids must include all
materials, labor, equipment, permits, and
associated work necessary for complete and functional
occupancy.

2. PROJECT OVERVIEW
Project Title: Tyler County EMS Station 60 –
Middlebourne
Owner: Tyler County Commission / Tyler County EMS
Location: Middlebourne, WV (exact site address to be
provided upon request).Type of Construction:
Turn-Key Modular Structure with
Attached Garage

3. SCOPE OF WORK
The selected contractor shall provide all services,
materials, and equipment necessary to complete
the project turn-key ready,
including but not limited to:

A. Modular Home / Living Quarters

Minimum 2,500 square feet on the main floor

Four (4) bedrooms and two (2) full bathrooms

Full kitchen with cabinetry, counters, and included
appliances

Living room area
Second-story storage space (accessible via stairs)

All appliances included: refrigerator, stove/oven, dishwasher, washer, and dryer

Vinyl siding and asphalt shingles to match the
garage structure

Handicap accessible ramp

B. Foundation and Site Work

Complete foundation system for both home and
garage

Site preparation, grading, and all necessary
groundwork

Septic system design and installation, including
permits and approvals

All utility hookups (electric, water, sewer/septic, gas,
etc.)

Asphalt-paved driveway and parking area a 30 feet heated concrete apron running the length of the garage bay.

C. Attached Garage / Apparatus Bay

Total size: 48 feet x 55 feet (2,650 sq. ft.)

Three (3) 14-foot overhead garage doors with
windows and man doors

Block walls (4 feet high) with drywall/metal finish
above

8-inch reinforced concrete floor with a floor drain
running full length

Radiant heat gas tube heaters

LED lighting system throughout

Vinyl siding and shingles to match the home.

Electric and water outlets/faucets installed

36-inch fire-rated doors

4. GENERAL REQUIREMENTS

All materials and workmanship must meet or exceed
West Virginia building codes and local zoning
regulations

Contractor responsible for all permits and required inspections

Contractor must provide proof of insurance, WV contractor’s license, and bonding capability

Project to be delivered turn-key ready for immediate occupancy

Contractor responsible for site cleanup and debris
removal upon completion

5. BID SUBMISSION REQUIREMENTS
Bidders must submit a sealed bid package including:

1. Completed Bid Form with total project cost and
line-item breakdown

2. Estimated project timeline (start and finish dates)

3. List of subcontractors (if applicable)

4. Proof of insurance, licensure, and bonding
capacity

5. Three (3) references for similar completed projects

All bids must be clearly marked:
“Bid Proposal – Tyler County EMS Station,
Middlebourne WV”

6. EVALUATION CRITERIA
Bids will be evaluated based on the following:
Compliance with specifications and scope
Total project cost
Project timeline
Contractor qualifications and experience
Warranty and service terms

The Tyler County Commission reserves the right to
accept or reject any or all bids, waive informalities, and award the contract in the County’s best interest.
