NOTICE TO ALL CORPORATIONS, ASSOCIATIONS, SOCIETIES, ORGANIZATIONS, INSTITUTIONS AND/OR TRUST LOCATED IN OR OPERATING IN LEWIS COUNTY, WEST VIRGINIA, OR WHICH BONA FIDE MAY MANIFEST A WILLINGNESS TO LOCATE OR OPERATE IN SAID COUNTY, AND WHICH ARE DEVOTED TO A CHARITABLE PURPOSE OR WHO ARE OR MAY BECOME ENGAGED IN PROVIDING AND MAINTAINING FACILITIES FOR THE MEDICAL, CLINICAL OR HOSPITAL CARE AND TREATMENT OF AFFLICTED NEEDY OF SAID COUNTY, WHO DESIRE TO BE CONSIDERED FOR PARTICIPATION AS BENEFICIARIES OF THE NET INCOME AVAILABLE FOR DISTRIBUTION DURING THE PERIOD FROM JANUARY 1, 2026 TO DECEMBER 31, 2026, BOTH DATES INCLUSIVE, FROM THE TRUST ESTABLISHED UNDER THE WILL OF MELVIN BURR SPRIGG, DECEASED: Pursuant to the last will and testament of Melvin Burr Sprigg dated May 22, 1952, as interpreted by decree of the Circuit Court of Lewis County, West Virginia, in Civil Action No. 432, entered on December 16, 1964, please file, if you so desire, with the undersigned trustee on or before June 1, 2026, at its office at 466 Chestnut Ridge Road, Morgantown, West Virginia, 26505, written application to participate as beneficiary in the net income for the said period, stating the category you consider yourself eligible, whether you are presently active in Lewis County, verified under oath or by affirmation, supported by the following information. This application can also be received in electronic form by contacting [email protected]. Grant applications will be reviewed upon receipt. Once all required information is received, the proposal will be reviewed for consideration. Additional information may be requested, including the following: a. True copy of a letter from the Internal Revenue Service (or such other evidence satisfactory to trustee) showing that you come within the guidelines of those organizations, and agencies enumerated by the Internal Revenue Code of the United States of America that gifts and transfers to which are exempt from Federal Estate Tax. b. Financial report of all receipts and disbursements for the calendar year 2025 (or fiscal year ending within the said calendar year) properly certified by a practicing public accountant. The report should summarize all expenditures made for charitable purposes in Lewis County, West Virginia. c. If you seek to participate in the income aforesaid for providing and maintaining facilities for medical, clinical or hospital care and treatment for the sick and afflicted needy of said Lewis County you shall, in lieu of the report of receipts and disbursements required in subparagraph (b), provide the following (but without providing any names or identifying information of persons): (1) List of county(ies) of residence of persons so treated for the preceding year, if any: Said Trustee reserves the right to require such additional information as to status of exemption from Federal Estate Taxes and charitable purposes or provision for the sick and afflicted needy, as it deems necessary.
