*Pursuant to WV Code§ 48-25-l0l(b) the Petitioner has represented to the Court that the name change is being sought to protect identity of the Minor for personal safety reasons and therefore is not required to set forth the name to which his/her name will be changed.
Please take notice that on the 26th day of June, 2026 at 11:30 a.m., in Monongalia County, West Virginia before Judge Scott, Jessica McCormick will apply to said Circuit Court for permission to legally change the above-named minor’s name as such.
Any person having any objection(s) to said change of name shall appear at the time, place and date aforesaid and make his/her objection(s) known. Please note that the hearing may be rescheduled without further notice or publication pursuant to Chapter 48, Article 25, Section 101 of the West Virginia Code.
